The back indicates that it's a "SUPERCHROME" color product by Bill Bard Associates of Monticello, New York, and Miami Beach, Florida.
Size: approx. 6" x 4.25"
Date: Not dated, but I know that it's from the late-1990s or earlier.
Condition: Fine, I'd say. The card is unused and clean, with no noticeable edge wear and just barely bumped corners. (Please see scans.)
